Stull Orthodontics Scholarship Program

Dr. Jeri Stull, and Team would like to invite you to participate in the Stull Orthodontics Scholarship Program.  The program offers three $1,000 scholarships to graduating seniors from the local area.  This opportunity is available to any student in Campbell, Kenton, Boone, Pendleton, and Bracken counties, as well all patients of Stull Orthodontics in Kentucky, Ohio, and Indiana.  Winners will be notified by mid-May to assist in the student’s college decision. 

Applications can be obtained from:
     -   the office of Stull Orthodontics
     -   a high school guidance counselor

In order to be eligible for the scholarship, students must submit:

1. A Stull Orthodontics Application form (click here to download. You will need Adobe Reader to view and print the Application)
2. Their High School transcript and recent SAT I and/or ACT scores
3. Letters of recommendation from a guidance counselor, teacher and/or high school principal
4. A personal reference from a non-family member
5. Any other information which might be helpful or will aid an application
6. A signed parental consent form 

The deadline for the Stull Orthodontics Scholarship Program is March 26, 2010.
